Portal Venous Branching of the ¥³ Segment of the Liver

Abstract
Background/ Purpose: Several authors have described portal venous branches of the left medial segment(LMS) ranging from 1 to 10. However, the exact pattern of portal venous branching has not been readily identified. This study was aimed at describing an integrated concept of portal venous branching of LMS.

Methods: Forty portal-phased spiral CT films were serially examined to determine the portal venous branching of LMS.

Results: Horizontal portal branches arising from the umbilical portal vein(UPV) were seen at or slightly above the level of hilum. These included a main branch(M,100%) running to the right and slightly antero-superiorly, another branch running anteromedially(4,36/40,90%) and a third blanch directed posteriorly(P,14/40,35%). There were several upward branches from both proximal and distal portion of their horizontal branches(Sh,100%) and from UPY Su,29/40,73%). Several inferior branches were observed to arise from horizontal branches(Ih,17/35,49%) and from UPY(Iu,23/35,66%). Umbilical hepatic vein(UHV) running above UPV was observed in 15(38%).

Conclusion: The main and other horizontal branches seem to divide the LMS into superior and inferi or subsegments, and the superior branches arising from the proximal part of horizontal branches and UPV seem to supply the most supero-medial portion ol LMS. UHV seems to be another landmark dividing left lateral and medial segment.
